Student health requirements
 HEALTH APPRAISAL FORM

Dear Parents:
The New York State Education Department requires health appraisals (physicals) for students who:

  1. enter the school district for the first time
  2. are in Grades K, 2, 4, 7, and 10
  3. participate in interscholastic sports in grades 7-12

Health appraisals are acceptable if administered not more than twelve months prior to the beginning of the school year in which the examination is required.

We suggest that the health appraisal be done by the family physician as he/she is the one who best knows the family. He/she is better able to judge any change or deviation in the child's state of health and can discuss any recommendations directly with the parents at this time.

If a health appraisal form is not returned before school physicals in October in the specified grades, students will be scheduled to be examined by a school physician.

It is recommended that parents have children examined each year by the family physician whether this is required for school or not. Please have the health appraisal form returned to school if this is done. DENTAL HEALTH CERTIFICATE


The New York State Education Department requests a dental health certificate for students who:

  1. enter the school district for the first time
  2. are in Grades K, 2, 4, 7, and 10

The new dental health certificate form developed by the State may be found here.

Students are requested to furnish a dental health certificate at the same time that health appraisal forms are required. Certificates must be signed by a licensed dentist. A list of dentists to which children who need comprehensive dental examinations may be referred for treatment on a free or reduced cost basis is available upon request at your child's school.

Please note the dental health certificate is a request from the New York State Education Department. Unlike the health appraisal form, students who do not submit the dental health certificate will be allowed to attend school.

I have been asked a lot about the H1N1 virus as well as the Flu. In health class we talk about taking precautions not just because it is Flu season but always.
• The children learn to cover their coughs or sneezes with their elbows.
• They learn the importance of staying away from people who are sick.
• They are taught to wash their hands often with soap and warm water.
• If soap and water are not available, then they should use an alcohol-based hand rub.
• The importance of avoiding the touching of ones eyes, nose and mouth are stressed daily.
• We talk about keeping surfaces like bedside tables, surfaces in the bathroom, kitchen counters and toys for children clean by wiping them down with household disinfectants according to directions on the product labels.
• Also the need to throw away tissues and other disposable items used by someone who might not be feeling well is discussed.

We discuss the need to have at least 4 new tooth brushes or as we call them in health class “PLACK SMACKERS” every year.

REMEMBER HAND WASHING IS THE SINGLE MOST IMPORTANT THING YOU CAN DO TO PROTECT YOUR HEALTH
